Summary

Key Responsibilities Medicines Optimisation Support the delivery of safe and effective medicines optimisation services Support patients in understanding their medicines and promote adherence Identify patients who may benefit from medication reviews or other pharmacy services Assist with medicines reconciliation following discharge from hospital Clinical Support Assist in maintaining accurate medication records Support the management of medicines related queries from patients and healthcare professionals Work with GP practices to improve prescribing safety and quality Support the monitoring of high risk medicines under supervision Medicines Safety Assist with audits relating to prescribing and medicines management Help identify and report medication incidents and near misses Contribute to quality improvement projects Support compliance with national and local prescribing guidance Administrative Duties Maintain accurate electronic patient records Prepare reports and documentation as required Support stock management where appropriate Participate in data collection and performance monitoring Learning and Development Complete all requirements of the Pharmacy Technician Level 3 Apprenticeship Attend training provider sessions Maintain a portfolio of evidence Participate in regular supervision appraisal and reflective practice Work towards registration with the General Pharmaceutical Council (GPhC) Team Working Work collaboratively with GPs nurses pharmacists pharmacy technicians care coordinators and other members of the multidisciplinary team Participate in PCN and practice meetings where appropriate Develop positive working relationships with community pharmacies hospitals and other healthcare providers General Responsibilities Maintain patient confidentiality at all times Comply with GDPR information governance and NHS policies Work within professional boundaries and seek support when required Promote equality diversity and inclusion Maintain mandatory training requirements Contribute to a safe working environment and follow health and safety procedures Demonstrate the NHS values and behaviours in all aspects of work
