Summary

A Vacancy at Wirral University Teaching Hospital NHS Foundation Trust.

An exciting opportunity has arisen for an enthusiastic and highly motivated individual to join the Acute Oncology Team, within the Medical Division of Wirral University Teaching Hospital NHS Trust. The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life. The aim of the role is to make sure that people living with cancer can access the right support, at the right time.

 The post-holder will take an active role in contributing towards the development and Implementation of cancer survivorship initiatives enabling people affected by cancer to navigate the health and social care system and become self-supportive i.e. coordinating health and well-being clinics to enable independence and support self-management as appropriate. 

The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs.
