Summary

Are you a compassionate and motivated Registered Nurse looking to make a real difference in your local community? We are looking for a Community Staff Nurse to join our friendly and supportive Community Nursing Team.

In this rewarding role you will provide skilled nursing care to patients in their own homes and community settings, helping them to maintain their independence and achieve the best possible outcomes. Working under the guidance of a District Nurse, you will build meaningful relationships with patients and their families, delivering personalised care with kindness, dignity and respect.

This is an excellent opportunity to develop your community nursing skills, work closely with a wide range of health and social care professionals and contribute to the ongoing development of services that make a positive impact on people's lives.

As a Community Staff Nurse, you will:

• Deliver high-quality, person-centred nursing care to patients in their own homes and community settings.

• Assess, plan, implement and evaluate care to meet individual patient needs.

• Work in partnership with patients, carers and multidisciplinary colleagues to coordinate effective care and support.

• Promote the highest standards of care, compassion and professionalism in everything you do.

• Support the coordination of the service during the short-term absence of the District Nurse.

• Work closely with GPs, practice teams, palliative care services, social care teams and voluntary agencies to ensure seamless patient care.

• Support and encourage colleagues, learners and support workers within the team.

• Contribute to service development, quality improvement initiatives and the advancement of community nursing practice.

• Maintain and develop your clinical knowledge and skills through ongoing professional development.
