Job Description

We are looking to recruit a motivated and experienced Care Manager to join our team in sunny Deeside, delivering a Priority Discharge Care Management Service primarily based at Glen O'Dee Hospital, Banchory, with the flexibility to provide cover at Aberdeen Royal Infirmary when required.

This is an exciting opportunity to work within a supportive, collaborative and dynamic multidisciplinary team, playing a key role in facilitating safe and timely hospital discharges and supporting positive outcomes for individuals and their families.

The post offers a Monday to Friday working pattern, providing an excellent work-life balance while contributing to the delivery of high-quality health and social care services.

There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the Service and the organisation.

Requirements
