Summary

Prestige Nursing & Care East Lancashire is looking for a compassionate, organised and proactive Senior Care Coordinator to join our Blackburn office team. This is a key operational role within the branch: bringing together people, information and care arrangements so clients receive reliable, person-centred support.

We provide high-quality home-based care across East Lancashire, including specialist and complex packages delivered in partnership with families, case managers, commissioners and clinical professionals. The role combines office-based coordination with visits to people in their own homes, making it a strong opportunity for someone who enjoys solving problems, building trusted relationships and keeping several priorities moving.

What you will be doing

  • Create and maintain safe, effective staff rotas using IRIS Digital Care, matching the right colleagues to each client's needs.
  • Coordinate new referrals from initial information through assessment, mobilisation and a well-organised start of care.
  • Carry out care assessments, reviews and manual-handling reviews in people's homes, within your training and competence.
  • Keep training, compliance and care documentation accurate, current and ready for review or audit.
  • Support the recruitment, on boarding and day-to-day engagement of care colleagues.
  • Be a responsive point of contact for clients, families, case managers, commissioners and other professionals.
  • Spot risks or service pressures early, take practical action and escalate appropriately.
  • Take part in the shared out-of-hours on-call rota with support from the wider branch team.

What success looks like

Clients receive reliable, person-centred care from colleagues with the right skills; rotas, records and compliance information stay accurate; families and care colleagues feel informed and supported; and new packages are mobilised safely and promptly.

What will help you succeed

  • Experience in care coordination, rostering or a senior care role, with a sound understanding of home care.
  • Excellent organisation, administration and communication skills, including confidence using digital systems.
  • A calm, practical and solution-focused approach when priorities change or pressure increases.
  • Working knowledge of person-centred care, safeguarding, risk management and CQC expectations.
  • Confidence building professional relationships while handling sensitive information appropriately.
  • A full UK driving licence and access to your own vehicle, as regular travel to client's homes is an essential part of the role.

Useful, but not essential

  • Previous experience with IRIS Digital Care or another care-management and rostering system.
  • Experience completing care assessments, reviews or manual-handling risk assessments.
  • A Level 3 adult-care qualification, or willingness to work towards relevant development.
