Summary

The post holder will be responsible for delivering a broad range of general practice nursing services, including health promotion, preventative healthcare, immunisation and vaccination, wound care, screening and long-term condition management. The Practice Nurse will work collaboratively with GPs, Healthcare Assistants, Clinical Pharmacists, First Contact Physiotherapists and administrative staff to ensure patients receive safe, effective, compassionate and appropriate care. The post holder will work within their professional competence and the NMC Code, recognising when further advice, support or clinical escalation is required. The Practice Nurse will undertake a varied programme of clinical consultations and procedures, including: General nursing assessments and consultations. Routine observations, including blood pressure, pulse, oxygen saturations, temperature, weight and BMI. New patient health assessments. NHS Health Checks. Health promotion and lifestyle advice. Cardiovascular risk assessment. Smoking cessation and healthy lifestyle support. Wound assessment and management. Dressings and wound care. Suture and clip removal. ECG recording. Urinalysis and appropriate investigations. Blood sampling where trained and competent. Spirometry and respiratory assessments where appropriately trained. Ear care where appropriately trained. Supporting minor illness and appropriate escalation of patients where required. Patient education and self-management advice. Comply with the practice Infection Prevention and Control policy. Follow appropriate hand hygiene procedures. Use PPE appropriately. Maintain safe clinical environments. Dispose of clinical waste and sharps appropriately. Maintain appropriate standards of cleanliness and infection control within the treatment room. Report infection control concerns to the appropriate person. The Practice Nurse will actively promote preventative healthcare and support patients to improve their health and wellbeing. This may include: Healthy lifestyle advice. Weight management. Smoking cessation. Alcohol awareness. Exercise and physical activity. Cardiovascular risk reduction. Diabetes prevention and management. Respiratory health. Vaccination awareness. Screening programmes. Self-management of long-term conditions. Participate in infection control audits where required. The Practice Nurse will contribute to the practice's clinical governance programme by: Participating in clinical audits. Contributing to quality improvement projects. Participating in significant event reviews. Identifying opportunities to improve patient care. Following evidence-based clinical guidance. Supporting implementation of national and local healthcare initiatives. Maintaining awareness of relevant changes in primary care. Contributing to practice meetings where appropriate. Supporting the practice to meet CQC, NHS and contractual requirements.
