Summary

We are delighted to offer an exciting opportunity for a Clinical Pharmacist to join our fast-paced, progressive and forward-thinking primary care team. This is a rewarding and varied clinical role for a pharmacist who is passionate about improving patient care and wants to make a meaningful contribution within a multidisciplinary environment. You will have the opportunity to use your clinical expertise to optimise medicines, support patients with complex and long-term conditions, and help shape the delivery of pharmacy services across the practice. We are looking for someone who is proactive and patient-focused, with the ability to work independently while also being an enthusiastic and collaborative member of the wider team. As a Clinical Pharmacist, you will play a key role in ensuring the safe, effective and evidence-based use of medicines across the practice. You will work across both patient-facing and clinical activities, using your professional judgement to identify opportunities to improve treatment, support better outcomes and promote medicines optimisation. Your responsibilities will include: Managing acute and repeat prescription requests, ensuring prescribing is safe, appropriate, cost-effective and consistent with current national and local guidance. Delivering Structured Medication Reviews (SMRs) and providing personalised support to patients with long-term and complex health needs. Working with patients who may have multiple conditions or medicines, including those living with chronic pain, hypertension and diabetes. Conducting patient consultations and reviewing clinical records, medication histories and prescribing to identify risks, optimise treatment and improve outcomes. Applying current NICE guidance, QOF requirements and local prescribing policies to support high-quality, evidence-based clinical care. Providing specialist pharmaceutical advice to GPs, nurses and other members of the multidisciplinary team. Contributing to clinical discussions and supporting the MDT with complex medication-related decisions. Supporting the safe and effective processing and management of shared care agreements, helping to ensure continuity of treatment between primary and secondary care. Providing clinical supervision, guidance and support to pharmacy technicians within the practice. Supporting the learning and development of trainee pharmacy technicians and trainee pharmacists, helping to create a positive environment for professional development. Participating in clinical audits, quality improvement and service development projects, identifying opportunities to improve patient safety, medicines use and the overall quality of care. Keeping clinical knowledge and professional practice up to date and actively contributing to the development of the practice pharmacy service. What We Are Looking For We are seeking a pharmacist who is enthusiastic about primary care and committed to delivering high standards of patient-centred care. You will be able to demonstrate: Strong clinical knowledge and sound professional judgement. A proactive and solutions-focused approach to patient care. Excellent communication and interpersonal skills, with the ability to build positive relationships with patients and colleagues. The confidence to work independently and make appropriate clinical decisions. Strong organisational and time-management skills, with the ability to manage competing priorities in a busy environment. A collaborative approach and a genuine interest in working as part of a multidisciplinary team. A commitment to continuous learning, clinical development and service improvement. Experience of working in primary care would be advantageous but not essential, and familiarity with SystmOne is desirable. Why Join Us?
