Summary

We are looking for a compassionate and motivated Community Assistant Practitioner to join our therapy team, supporting patients in their own homes, care homes, and community settings. Working as part of an integrated multidisciplinary team, you will assist in delivering rehabilitation programmes, support individuals living with long-term conditions, and contribute to palliative care interventions. Your role will focus on helping patients regain and maintain independence, achieve their personal goals, improve their quality of life, and reduce the need for unnecessary hospital admissions. Previous therapy experience is desirable.

• To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community, Community Hospitals, and clinics, by assessing, planning, implementing and evaluating packages of care.

• To work within a reablement philosophy, using strengths-based assessments and actively promoting independence for the patients.
