Summary
Help Build the Future of Urgent Community Care The Community Access Service (CAS) Hub is at the forefront of transforming urgent and community care across Mersey Care NHS Foundation Trust. Following a successful launch in Knowsley, the service is now expanding to support patients and clinical teams across Liverpool, South Sefton, Knowsley, St Helens and Halton, creating a truly integrated front door for urgent community services.
We are looking for enthusiastic and talented professionals to join us at an exciting stage of development. This is a unique opportunity to influence how services are designed, delivered and continuously improved, whilst working alongside a highly skilled multidisciplinary team.
Working within the CAS Hub, you will support the coordination, assessment and navigation of referrals, helping ensure patients receive the right care, in the right place, at the right time. You will play a key role in supporting admission avoidance, improving patient flow, and enhancing the experience of both patients and referring professionals.
As the service continues to expand across multiple localities, successful applicants will have the opportunity to contribute to service development, innovation and quality improvement, helping shape the future model of integrated urgent community care across the Mersey Care footprint.

The post holder will support the operational development and coordination of the Community Access Service (CAS) Hub as it expands across Liverpool, South Sefton, Knowsley, St Helens and Halton. Working across organisational and professional boundaries, the role will contribute to mobilisation activities, pathway development, demand and capacity management, performance monitoring, service improvement initiatives and the implementation of a single integrated front door for urgent community care services. This is an exciting opportunity to play a key role in shaping a new model of care across the Mersey Care footprint.

Professional
Maintain current professional registration.

Be involved and committed to Clinical Supervision and act as a reflective practitioner
Maintain personal professional development by completing requirements of PREP, actively participating in Personal Development Reviews, completing mandatory training and attending staff
meetings.
Provide evidence-based practice and a high quality integrated care services tailored to the needs of individuals within the ICRAS . These services will be delivered within appropriate environments.
Management
The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders, from a wide range of services and disciplines.
The post holder will coordinate complex activities that span a range of disciplines and organisations both NHS and non –NHS.
Record, interpret and present health information and issues that can impact on the risk or needs assessment of patients from a range of health perspectives. This may involve disclosing relevant
information on the patient or carers in ensuring the information is processed fairly and proportionally.
Develop and implement data collection systems and ICRAS protocols that will provide accurate and timely data safely and effectively and within the bounds of confidentiality and the data protection
act.
Disseminate and communicate decisions and appropriate health information to relevant health professionals including primary and secondary care for sharing best practice.
Identify service gaps and issues relating to effective clinical pathways and /or cross boundary arrangements.
Escalate evidence of ineffective arrangements within or outside of the ICRAS.
Records appropriate information about health outcomes on the appropriate recording system on a case-by-case basis.
Participate in relevant internal and external working groups/projects, services and initiatives to provide, information and analytical advice and expertise.
The post holder will carry out any other duties as may reasonably be required by their line manager.
Leadership
Contribute to performance improvement of the ICRAS taking the lead for identified areas where agreed by the team
Provide coordination of and participate in relevant internal and external working groups and provide advice, expertise and support where requested.
Provide relevant and timely advice and reports on the function of the ICRAS to Urgent and Intermediate Care service line within Community Division
Work with members of the ICRAS team to investigate the causes of any variance in ICRAS activity and proactively contribute to the implementation of solutions.
Participate and lead specific working groups in order to achieve targets and objectives in relation to ICRAS
Proactively participate to identify risks and issues with regards to the functions of ICRAS teams to facilitate resolution and /or escalation processes.
Clinical
Act as a resource on matters of ICRAS care for other disciplines, agencies and the general public.
Prioritise and manage time and workload effectively, informing line manager about workload constraints affecting services.
Be responsible for identified team members in their role, responsibility and professional development.
Provide within the team, support and advice in areas of professional expertise.
Support staff in the team in developing skills within the Scope of Professional Practice.
The post holder shall as necessary provide cover for and undertake duties of absent care coordinator
colleagues across Liverpool and South Sefton ICRAS teams.
The post holder shall follow all the policies and procedures for the organization.
Provide clinical leadership as appropriate.
Non Clinical
Provide regular reporting and monitoring from a quality and safety perspective.
Contribute to the strategic planning of ICRAS team projects, identifying interdependencies across
projects/functions and services, potential impacts of integrated care activity on wider organisation,
resource requirements and building in contingency and adjustments as necessary.
Contribute to the development of performance and governance strategies and the development and implementation of integrated care improvement programmes in accordance with Trust
priorities.
Contribute to short, medium and long term service developments, for ICRAS to develop and achieve quality outcomes.
Manage own workload and make informed decisions in the absence of required information, working to tight and often changing timescales.
Use computer software to create reports for other professionals and meetings as required. Develop policies and procedures in own work function with an impact on the wider organisation, as required.
Contribute to the drawing up of the ICRAS team budget.
The post holder will hold a designated budget and be responsible for the purchase of physical assets and supplies.
Training & Education
Participate in an individual performance review on an annual basis and be proactive in Continuous
Professional Development.
Utilise informal and formal learning opportunities both independently and with others.
Take a lead in the coordination and provision of training and development, to ensure a wide understanding of the ICRAS.
Provide internal and external training development of the ICRAS team.
Practice in accordance with the professional, ethical and legal codes of the Nursing and
Midwifery Council and Mersey Care NHS Foundation Trust protocols and guidelines.
Be accountable for own practice.
