Are you looking for a role where no two days are the same?

Our Hospital Social Care Team at Lincoln County Hospital is looking for a Community Care Officer to join us.

You will be part of a busy, supportive multidisciplinary team, working alongside social workers, occupational therapists, nurses, therapists and health colleagues to help people leave hospital safely and return to living as independently as possible.

What will you be doing?

You will manage a supervised caseload of people with a range of care and support needs. You will work with people, their families and professional networks to understand what matters to them, identify their strengths and find practical solutions that support their independence.

Your role will include:

Completing assessments and identifying people's care and support needs.

Working in a strengths-based way to achieve positive outcomes.

Helping to arrange and develop personalised care and support packages.

Providing information and advice to people and their families.

Working with health, housing, voluntary and independent sector colleagues.

Supporting qualified social workers, therapists and nurses.

Identifying and managing risk, including responding to safeguarding concerns.

Arranging equipment and minor adaptations where appropriate.

Monitoring and reviewing care and support arrangements.

Contributing to multidisciplinary and multi-agency meetings.

Keeping accurate records and using information to monitor progress and performance.

Hospital-based, but not limited to one hospital

Although you will be based at Lincoln County Hospital, our team also supports hospital discharges from Northern Lincolnshire and Goole NHS Foundation Trust (NLAG) hospitals, including Grimsby Hospital, Scunthorpe General Hospital, Goole and District Hospital and Diana, Princess of Wales Hospital.

This means that travel to other hospital sites may be required as part of the role.

You will have the opportunity to work across health and social care, building strong professional relationships and helping people move from hospital back into their communities.

When will you work?

This is a 30-hour permanent position.

Our usual working hours are:

Monday to Thursday: 8.45am to 5.15pm

Friday: 8.45am to 4.45pm

The role requires a good standard of general education, including English and Maths, with a minimum of four GCSEs at Grade C or above or equivalent.
