We’re looking for a motivated, organised, and compassionate CareCoordinator to join our collective Integrated Community Teams with partner organisations — a dynamic group of clinicians, practitioners, and community partners all dedicated to delivering outstanding, joined‑up care.

As a Care Coordinator, you’ll be at the heart of our multidisciplinary team (MDT), ensuring everyone stays connected and informed. Your role will include:

• Coordinating MDT meetings and streamlining communication

• Supporting proactive, personalised care planning for patients with complex needs

• Working closely with GPs, nurses, therapists, social care, and voluntary sector partners

• Helping the team deliver seamless, patient‑centred care across our neighbourhood

• Keeping systems updated and making sure the right information flows to the right people at the right time
