Summary
The Lead Practice Nurse will provide clinical leadership and professional support to the practice nursing team, ensuring the delivery of safe, effective and high-quality nursing care. The post holder will maintain an appropriate clinical caseload and work autonomously within their professional scope of practice. Clinical responsibilities will include chronic disease management, cervical screening, immunisations and vaccinations, wound care, health promotion, ECGs and other treatment room procedures appropriate to their competencies. The Lead Practice Nurse will take responsibility for the day-to-day clinical leadership of the nursing team, providing advice, guidance and professional oversight and promoting consistent standards of care. The post holder will lead and support the management of long-term conditions including asthma/COPD, hypertension, diabetes and cardiovascular disease, and will contribute to the achievement and monitoring of QOF and other relevant primary care quality requirements. The role will include coordinating vaccination and immunisation programmes, including seasonal and national campaigns, ensuring appropriate clinical guidance and Patient Group Directions are followed and overseeing vaccine stock, storage and cold-chain requirements. The post holder will act as the clinical lead for infection prevention and control, supporting audits, reviewing procedures and ensuring appropriate standards are maintained throughout the practice. Working closely with the Practice Manager and GP Partners, the Lead Practice Nurse will contribute to the development and review of clinical policies, protocols and systems and support the effective planning and delivery of nursing services. The post holder will participate in and lead clinical audits, service evaluations and quality improvement initiatives, identifying opportunities to improve patient care and nursing services. Responsibilities will also include oversight of clinical supplies, equipment and vaccine stocks, ensuring appropriate systems are in place for ordering, stock rotation, safety checks and minimising waste. The Lead Practice Nurse will support the effective management of the nursing team, including workload, capacity, skill mix, supervision, competency and professional development. The post holder will work collaboratively with GPs, the Practice Manager, the wider practice team, Primary Care Network and community healthcare services to support effective and joined-up patient care. All clinical activity must be accurately and contemporaneously recorded on SystmOne. The post holder will maintain current NMC registration, work in accordance with the NMC Code and ensure professional, clinical governance, confidentiality and information governance requirements are maintained. The post holder will be expected to maintain their own professional knowledge and competencies and remain up to date with relevant national guidance and developments within General Practice.
