Summary
Conducting structured medication reviews and supporting patients with long-term conditions Reviewing and optimising medicines to improve patient outcomes and safety Managing medication-related queries from patients, GPs, nurses, and wider healthcare teams Completing clinical medication tasks, including prescription queries and medicines reconciliation Providing advice on prescribing, medicines safety, and appropriate use of medications Identifying opportunities to improve prescribing quality and reduce medicines-related risks Supporting audits, quality improvement projects, and medicines optimisation initiatives Working collaboratively with GPs, practice teams, community pharmacy, and other healthcare professionals Acting as a clinical resource for medication-related advice across the PCN
