Summary

About you You will: Speak to individuals to who are on the SMI (Severe Mental Illness) register, describe the benefits and what happens at a Physical Health Check and enable their appointment. Develop a rapport with individuals and identify barriers to accessing services and support to attend local services and interventions that could improve their physical health. Contact individuals after their Physical Health Check, discuss the results and any recommendations for improving their physical health. Work autonomously across the Locality to implement the service and drive its success. Build relationships with stakeholders, particularly Primary Care Networks, increase awareness of the service and codesign processes across the Locality. Promote recovery, inclusion, and equity in everything you do. Youll need: A minimum of one year experience working with people with mental health needs, or people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency and offending). Proven experience of identifying risk, effective documentation and escalation of risk concerns. Strong communication and interpersonal skills. Ability to manage caseloads, work flexibly, and build effective multi-agency partnerships. Confidence in IT systems and record-keeping. A deep understanding of recovery and trauma-informed practice. A current, full driving licence and access to appropriate motorised vehicular transport (such as a car, moped or motorbike).
