Summary
CNWL Pharmacy is seeking an enthusiastic, experienced and motivated Advanced Specialist Pharmacist to join our team in Milton Keynes, providing specialist clinical pharmacy and medicines optimisation support across Child and Adolescent Mental Health Services (CAMHS) and the Crisis Resolution Home Treatment Team (CRHTT).
This is an exciting opportunity to take a senior clinical role across two important mental health services, supporting patients with complex needs while working closely with multidisciplinary teams to deliver safe, effective and person-centred care.
• Provide a comprehensive specialist clinical pharmacy and medicines optimisation service to CAMHS and CRHTT patients and their multidisciplinary teams.
• Undertake patient-centred clinical medication reviews for people with complex mental health needs, polypharmacy, adherence difficulties and comorbidities.
• Provide expert advice on the safe, appropriate and cost-effective use of medicines.
• Participate in MDT meetings, inpatient ward rounds, CPA meetings and discharge planning where appropriate, particularly for patients transferring to CRHTT.
• Support reviews of pharmacological treatment options and, where appropriate, lead aspects of medication-focused ward rounds or MDT discussions.
As the Advanced Specialist Pharmacist, you will provide expert clinical pharmacy input across the CAMHS and CRHTT service lines. You will undertake comprehensive medicines reviews, provide specialist advice and contribute to the development of innovative medicines optimisation services.
You will work as an integral member of the multidisciplinary teams, collaborating with Consultant Psychiatrists, nurses, psychologists, occupational therapists, social workers and other healthcare professionals, as well as patients, carers and external partners.
The role provides an excellent opportunity to influence clinical practice, improve medicines safety and develop pharmacy services across community and home-treatment pathways.
We are looking for a pharmacist who can demonstrate:
• Significant clinical pharmacy experience and a strong commitment to patient-centred care.
• Specialist knowledge and experience relevant to mental health pharmacy and medicines optimisation.
• The ability to work confidently and collaboratively within multidisciplinary teams.
• Excellent clinical reasoning and decision-making skills when managing complex patients.
• Strong communication and interpersonal skills, including the ability to work effectively with patients, carers and healthcare professionals.
• Experience of supporting and developing colleagues through education, training, supervision or mentorship.
