Summary

This is a system-wide post with a focus on provision of on-site clinical and professional support to partners across the footprint of North Cambridgeshire & Peterborough Care Partnership - Fenland, Huntingdon and Peterborough. CUH is the host organisation and the employer. The system Consultant/Lead Pharmacist – Older People, also hosted by CUH, is the line manager. The purpose of the post is to:

 -      Support the Consultant Pharmacist in developing and delivering a strategy for care models that identify and manage older patients at greatest risk of severe medicine-related adverse effects.

-      Be accountable for the delivery of delegated workstreams in line with system and service priorities

-      Act as an expert resource in medicines optimisation in older people, liaising with the multidisciplinary team and system partners to promote safe, effective use of medicines in this patient group.

 -      Provide an advanced clinical pharmacy service to specific cohorts of older people.

 -      Collaborate with pharmacy teams from other providers and primary care to ensure service delivery to agreed standards in line with business planning requirements and NHS contracts e.g. DES

Honorary contracts will be facilitated to enable delivery of direct patient care to patients under the care of partner organisations where necessary e.g. North West Anglia NHS Foundation Trust (NWAFT) and Cambridgeshire and Peterborough NHS Foundation Trust (CPFT).
