Summary

Are you ready to make a real difference in diabetes care? We are looking for a passionate Specialist Diabetes Practitioner to join our forward-thinking team within the Northern Care Alliance. This is an exciting opportunity to work autonomously, delivering a high-quality, evidence-based service that improves patient outcomes and empowers people to effectively manage their diabetes.

In this role, you will assess, plan, and deliver individualised care while acting as a trusted source of advice and support for patients, carers, and healthcare professionals. You will also play a key role in shaping the future of diabetes services by providing specialist education and training to staff and students across all aspects of diabetes management.

In this autonomous role, you will provide a high-quality, evidence-based service for people living with diabetes, working within Trust policies, national guidelines, and your professional scope of practice. You will play a key role in improving patient outcomes by promoting best practice in diabetes care, reducing the risk of diabetes-related complications, and empowering individuals to take an active role in managing their condition.

You will be responsible for assessing, planning, implementing, and evaluating personalised care and treatment plans, ensuring patients receive timely access to appropriate care pathways and specialist referrals.

As a clinical expert, you will provide specialist advice and support to patients, carers, and healthcare professionals, while delivering education and training across multidisciplinary teams to enhance diabetes knowledge and skills.

You will also have the opportunity to initiate and adjust injectable and oral diabetes medications, supporting patients to manage their health and wellbeing more effectively.

The successful candidate will contribute to the ongoing development of the service through participation in clinical audit, quality improvement initiatives, and service evaluation activities.
