You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of adults and children.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

It is an exciting time to join primary care and community services as the national Ageing Well programme gathers pace.

DCHS is working closely with the Primary Care Networks to transform the care of housebound and vulnerable people across Derbyshire through our Team-Up approach.

We’re seeking an ACP with a special interest in the management of the typical conditions seen within a UTC, to provide day-to-day clinical leadership support to the teams.

To provide leadership support for the clinicians already working in the service, to advance the care of the presenting patients (in line with our 4 pillars approach).

We’re looking for ACPs with a proven track record of working at this level in an urgent care environment and who can support Service Leads and Unit managers in safe care delivery.
