Summary
Patient Care/Clinical Practice Work as part of a designated clinical team delivering care that focuses on the direct needs of the patient. Contribute to the planning of care delivery in association with the Registered Nurse. Contribute to on-going assessments recognising when it is necessary to refer to others for reassessment and action. Demonstrate the ability to recognise the deteriorating patient or other situations that require escalation to the appropriate professional for expert help and advice. Implement and evaluate individual care / treatment plans. Improve the quality of care by contributing to the continuous monitoring of patients care. Accept delegated responsibility from the Registered Nurse to monitor individual plans of care to meet the immediate and on-going needs of patients. Undertake a range of agreed clinical skills once trained and deemed competent including: Venepuncture and phlebotomy skills including HIV Point of Care Testing Urinalysis Pregnancy testing Taking patient observations and measurements including blood pressure, pulse, temperature, weight as required Staining and reading microscopy slides Managing and processing specimens Chaperoning and assisting clinicians with examination and procedures for male and female patients Care of HIV outpatients including blood taking, recording vital signs and physical details. Responsible for contributing to the clinical environment, which promotes a positive patient experience and care, promoting privacy and dignity at all times. Educate patients and families about sexual health & HIV related illness, conditions and medication under the direct / indirect supervision of the Registered Nurse. Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a patient and their family. Housekeep and Environmental Care To have an awareness of monitoring and maintaining stock levels of designated items. Keep all storage areas clean, tidy and in good order. Report any faulty or broken equipment to the ward manager and label appropriately. Deliver specimens, test requests, notes or any charts as requested. Escort patients to other departments as and when requested to do so. Promote and ensure safe handling of valuables and patients property as per Trust policy.
