Summary

Carry out nursing therapeutic work with complex patients with a learning disability and mental health disorders Work collaboratively with other members of the multi-disciplinary team (MDT) to ensure that treatment plans are in place and clear lines of accountability are established. This work will be undertaken in the context of multi professional and interagency working. In conjunction with the MDT, oversee the day to day delivery of treatment strategies to patients ensuring that all multi professional treatment recommendations are implemented and evaluated in a timely manner. Undertake comprehensive assessment and treatment planning and provides direct nursing interventions, advice and evaluation to the Secure Service ensuring that the care planning process does not delay the patient progress through the treatment pathway. Ensure that care plans are appropriate to the needs of the patients, person centred and reviewed in a timely manner that reflects the changing needs of the patient. Ensure the utilisation of evidence-based assessments, formulations and nursing therapeutic interventions, which appropriately meet the needs of patients on an individual and group basis
