Summary

Brief overview of the role, please see job description and person specification 1.Understanding of / and the ability to conduct assessments and review of care, care plans, and risk assessments, 117 reviews and continuing health care 2. To manage a caseload and to be allocated care coordinator, the applicant to have understanding /or experience of the role of the care coordinator. 3.Knowledge and understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983 and safeguarding, the NHS Constitution, and risk assessment / management 4.Experience in the performance management and supervision of lower banded staff, and able to deliver teaching and learning to lower banded staff. 5.Excellent IT skills i.e. Word / Excel, PowerPoint and Outlook 6.Flexible approach to work 7.Current driving licence, ability to travel across sites and across Trust boundaries to attend meetings, etc
