Summary

All general duties and responsibilities associated with an ACP or paramedics working within primary care. Clinical responsibilities In accordance with the practice rota, as agreed, the post-holder will be available to undertake a variety of duties comprising mainly visiting patients at home, but may include surgery consultations, telephone consultations and queries, care home ward rounds, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols, developing care plans for health. Actioning further management accordingly (arranging bloods or other investigations, referrals, prescribing as indicated, or admission if necessary). Provide clinical support to the Practice clinical teams, answering any clinical queries regarding patients seen who may require GP/ACP/Paramedic input. Providing counselling and health education. Referring to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. Expectation to complete a mandatory induction. Compiling and issuing computer-generated acute and repeat prescriptions. Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate. Communication Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Recognise the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Delivering a quality service Prioritise, organise and manage own workload in a manner that maintains and promotes quality, including participating in shared learning across the practice. Work as part of the team to seek feedback, continually improve the service and contribute to business planning. Managing information Use technology, SNOMED codes and appropriate software as an aid to planning, implementing and monitoring care, communicating information and reviewing and processing data for audit purposes. Learning and development Take responsibility for own learning and performance through continuous professional development, including participating in supervision, acting as a positive role model and assessing own learning needs. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice. Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (e.g. courses and conferences). Miscellaneous Work as part of the team to seek feedback, continually improve the service and contribute to business planning. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner. Duties may vary from time to time, without changing the general character of the post or the level of responsibility.
