Summary
Provide expert clinical assessment and diagnosis for patients presenting with undifferentiated, undiagnosed, and complex health problems. Initiate, evaluate, and review treatment plans, including prescribing medications and therapies within your scope of practice. Manage a range of acute, minor, and long-term conditions independently, referring to or liaising with GPs or secondary care where appropriate. Undertake comprehensive physical assessments, using advanced clinical skills and decision-making. Perform and interpret relevant investigations, such as blood tests, ECGs, and other diagnostic procedures. Promote preventative healthcare and offer patient education, self-care advice, and health promotion. Work collaboratively with the multidisciplinary team to ensure high standards of care, continuity, and efficiency. Contribute to clinical governance, audits, significant event analysis, and the development of local policies and protocols. Participate in service development and innovation, supporting quality improvement initiatives across the practice. Maintain accurate and timely patient records in accordance with legal, ethical, and professional standards. Support junior staff and students through mentoring, supervision, and education where applicable. Work within local & national policies, procedures and guidelines.
