Ideally the post holder will have experience of working in an acute hospital setting and be able todemonstrateclinical knowledge in caring for acutelyunwell patients. You will be responsible for planning, delivering and evaluating the personalised health and wellbeing care needs of patients, in line with the Trust’s Values & Nursing & Midwifery Professional Framework goals example: personal hygiene and comfort, vital signs and NEWS, early recognition and management of acutely ill patients, safe transportation, fluid balance monitoring, enteral feeding/ meeting nutritional needs, ECG, catheterisation, venepuncture, cannulation, blood transfusion, palliative care, care of PICC lines , admission and discharge, nasogastric tube insertion & care, care of IV infusion pumps, IVIG and cytotoxic infusions, psychological and spiritual support.

• Assess, plan, implement and evaluate care to meet patients’ needsin accordance withagreed local and national standards andevidence-basedguidelines.

• Support and provide patient education and healthpromotion

• Be responsible forthe delivery and co-ordination of care, throughappropriate professionalpractice, delegation and supervision of duties carried out by junior staff.

• Proactively manage risk andmaintainpatient safety, conforming to health, safety and security legislation, policies,proceduresand guidelines

• Assistin setting,monitoringandmaintainingstandards of care to improve the patient experience and outcome ofcare

• Communicate all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamlesscare

• Maintainaccurateand complete records of all care activities,in a timely manner, adhering to local and professional standards fordocumentation

• Act as a role model, presenting a positive image of self,teamand theorganisation

• Promote equality in care and practice by recognising,respectingand meeting the needs and choices ofindividuals

• Ensures environment of care is safe, wellequippedand compliant with standards for hygiene andcleanliness
