Summary
Work alongside Social Prescribers to help manage and assist aspects of their caseload and responsibilities. Play an active role (calls to patients) in data gathering for the pro-active care initiative Research new available resources using the internet, phone and face to face visits Maintaining the resources database Data entry to support reporting Manipulating our unique database to provide reporting Managing referral allocation daily and waiting list management Organising group session administration and patient communication Providing support to Portsmouth Social Prescriber Team by arranging meetings, minute taking, arranging speakers and training Managing budgets and keeping accurate records Designing newsletters for patients and internally General admin support for the team which can include referrals, general communication, diary management and training The role may involve innovative project work in line with new and future opportunities that are identified by Island City Network
