Summary

Paediatric Outreach Nurse

We are seeking a motivated Registered Children's Nurse to join the Paediatric Outreach Team based at Stoke Mandeville Hospital.

This role involves delivering high-quality, family-centred care to children and young people in their homes and community settings. The service supports children with acute and long-term conditions, complex healthcare needs and palliative care requirements, including end of life care.

Working within a multidisciplinary team, you will:

Deliver nursing care in home and community settings.

Support children and young people with acute, chronic and complex health needs.

Coordinate care for children with complex conditions.

Provide palliative and end of life care when required.

Work with families, carers, schools, primary care services and healthcare professionals.

Assess, plan, deliver and evaluate care.

The team works closely with the acute paediatric ward, providing seamless care across hospital and community settings.

We welcome applications from nurses with:

Registered Children's Nurse  qualification.

Experience in community children's nursing or caring for children with complex needs.

Strong communication and organisational skills.

A commitment to compassionate, family-centred care.

The ability to work independently and within a multidisciplinary team.

Service hours:

Monday to Friday: 8am to 6pm

Weekends and Bank Holidays: 10am to 3pm

An on-call end of life service is provided when required.

As a Paediatric Outreach Nurse, you will provide high-quality, family-centred care to children and young people with acute, chronic and complex health needs in home, community and clinic settings. Due to the close integration between the Paediatric Outreach Team and the acute paediatric service, staff may be required to support the inpatient setting during periods of increased acuity or service demand.

Key responsibilities include:

• Working autonomously and within a multidisciplinary team, using sound clinical judgement in complex situations.

• Assessing, planning, delivering and reviewing individualised care with children, young people and families.

• Providing specialist nursing care through home visits, clinics and telephone support.

• Delivering clinical interventions including intravenous therapy, vascular access care, enteral feeding management, oxygen therapy, suctioning, wound care and palliative care.

• Recognising and responding to patient deterioration and escalating concerns appropriately.

• Supporting timely discharge, preventing avoidable admissions and ensuring seamless care between hospital and community services.

• Managing workload effectively and responding to changing clinical priorities.

• Building therapeutic relationships and maintaining informed consent.

• Providing education and support to families, carers, schools and healthcare professionals.

• Contributing to safeguarding, service development, audit, quality improvement and continuing professional development.
