Summary

Prescribing Support Support the Practice Pharmacist and GPs with medicines reconciliation, including processing medication changes from hospital correspondence and new patient registrations. Assist with identifying and escalating medication discrepancies, safety concerns, or prescribing errors in line with clinical governance processes. Support medication reviews by gathering relevant information, preparing searches, and assisting with audit activity. Help deliver agreed prescribing improvements and medicines optimisation initiatives in line with national guidance e.g. NICE, MHRA and local prescribing priorities. Contribute to initiatives that reduce medicines waste and improve patient compliance. Liaison and Communication Act as a point of contact for medicationrelated queries from patients, pharmacies, care homes, and other healthcare professionals. Liaise with practice staff and the wider PCN multidisciplinary team to ensure consistent and efficient medicines management processes. Communicate clearly with patients and carers about the appropriate use of medicines, both verbally and in writing. Support practice communication by distributing medicines management information and assisting with patient letters or prescribing updates. Audit, Data & Reporting Support and manage data collection for medicines management audits and prescribing reviews. Monitor prescribing activity against national and local targets and contribute to the production of prescribing reports. Collate and interpret audit findings, make recommendations for improvement, and support implementation of postaudit Operational & Administrative Support Work jointly with prescription clerks to ensure robust recall and monitoring systems for highrisk medicines. Provide cover for prescription clerks as required, including processing repeat and acute prescription requests in accordance with practice protocols. Support the implementation and development of medicines management systems such as repeat dispensing schemes and pharmacyled services. Training, Quality & Development Support and deliver training for practice staff on prescribing systems and protocols, alongside the Practice Pharmacist. Contribute to the induction of trainees and learners by providing information on practice prescribing policies. Participate in audits, service development projects, and the development of prescribing policies and standard operating procedures. Engage in personal and professional development, appraisal, and ongoing training. Professional Standards, Safety & Confidentiality Maintain patient confidentiality and comply with data protection, equality, diversity, and health & safety policies at all times. Promote safe working practices and contribute to maintaining a safe and secure practice environment. Manage workload effectively, take responsibility for the quality of own work, and contribute to continuous service improvement. Team Collaboration Work closely with GPs, pharmacists, nurses, and other administrative staff to ensure safe and efficient prescribing processes. Participate in meetings, training, and service improvement initiatives related to medicines management. Provide support during audits and contribute to compliance with CQC and NHS standards. Other tasks The ability to work in a well organised and professional manner to meet the demands of the work, maintaining concentration and accuracy. Ability to work alone or as part of a team. Fire Warden and associated responsibilities. Chaperoning and assisting patients to consulting rooms Possess the ability to motivate, organise and prioritise your own workload. To undertake any other responsibilities, to maintain a high standard and efficient service.
