Job Description Job Title: Practice Nurse Practice: Park Medical Practice Hours: 26 to 28 hours per week Salary: £21.58 per hour (£42,196.73 FTE), equivalent to Agenda for Change Band 6 Reports to: Clinical Nurse Manager Professionally Accountable to: GP Partners Location: Chaddesden, Borrowash and University of Derby Clinical System: SystmOne Professional Registration: Current NMC registration required DBS Requirement: Enhanced DBS required Job Purpose The Practice Nurse will deliver high quality, evidence based nursing care across all three Practice sites. The post holder will provide treatment room services, long term condition management, cervical screening, travel health, immunisations and preventative healthcare, while contributing to QOF achievement, clinical governance, patient safety and continuous quality improvement. The post holder will work collaboratively with the Clinical Nurse Manager, GPs and wider multidisciplinary team to promote health, wellbeing and self management and to support the achievement of Practice objectives and enhanced services. Clinical Responsibilities The Practice Nurse will: Deliver evidence based long term condition reviews including diabetes, asthma, COPD, hypertension, chronic kidney disease and cardiovascular disease. Provide treatment room services including wound care, dressings, suture removal, venepuncture, ECGs, spirometry and NHS Health Checks. Undertake cervical screening in accordance with national guidance. Administer childhood, adult, seasonal and travel vaccinations in accordance with appropriate PGDs and PSDs. Provide health promotion, lifestyle advice and preventative healthcare. Maintain accurate patient records and clinical coding within SystmOne. Recognise deteriorating patients and escalate concerns appropriately. QOF and Long Term Conditions The post holder will: Support achievement of QOF indicators through the delivery of high quality clinical care. Use approved clinical templates and appropriate coding to ensure accurate data capture. Support effective patient recall systems and appropriate follow up of patients. Work collaboratively with the wider Practice team to support long term condition management and Practice objectives. Clinical Governance The Practice Nurse will: Participate in clinical audit and quality improvement activity. Comply with CQC standards and Practice requirements relating to safeguarding, infection prevention and control and information governance. Contribute to learning arising from complaints, incidents and Significant Events. Maintain high standards of patient safety and professional practice. Infection Prevention and Control The post holder will: Maintain high standards of infection prevention and control within treatment rooms and clinical areas. Follow cold chain procedures and requirements for safe vaccine management. Identify and report risks appropriately. Participate in and support infection prevention and control audits. Professional Responsibilities The Practice Nurse will: Maintain current NMC registration and meet all requirements for professional revalidation. Work in accordance with the NMC Code and within their individual scope of professional competence. Participate in mandatory and statutory training. Maintain continuing professional development appropriate to the role. Comply with Practice policies, procedures and clinical protocols. Financial and Resource Management The post holder will: Use clinical resources responsibly and appropriately. Monitor clinical stock and report shortages or concerns. Support the Practice in achieving value for money and reducing unnecessary waste. Communication and Working Relationships The Practice Nurse will maintain effective professional working relationships with GP Partners, the Practice Manager, Clinical Nurse Manager, nursing colleagues, Healthcare Assistants, reception and administration teams, PCN colleagues, community services, secondary care providers and patients. The post holder will communicate effectively with patients, carers and colleagues and contribute positively to multidisciplinary and team working across the Practice. General Requirements The post holder will: Comply with all Practice policies and procedures. Maintain patient confidentiality and comply with UK GDPR and information governance requirements. Promote equality, diversity and inclusion. Participate in annual appraisal and maintain an appropriate personal development plan. Maintain the professional standards expected of a Registered Nurse. Undertake any other reasonable duties commensurate with the role.
