Summary

Facilitate communication between patients, their relatives and the multi-professional team both within the organisation and with external stakeholders that results in clear responsibilities being identified, and ensuring that care is delivered both effectively and efficiently Communicate highly sensitive/ difficult information and subsequently support patients, relatives, carers and colleagues as appropriate. Impart distressing news to patient/ relatives in considerate and professional manner Use highly developed clinical knowledge to independently assess, diagnose, plan, implement and evaluate treatments and interventions for patients presenting to the specialty with complex undifferentiated or undefined presentations. Undertake physical examination as indicated by the patients condition; liaising with the wider MDT to develop the most appropriate treatment plans. Manage clinical events involving patients alongside the wider team, often in unpredictable circumstances and requiring high levels of mental and/or physical effort according to the patients dependency and clinical need. Acting as a role model and teacher, providing mentorship and support and demonstrate exemplary practice to junior members of the healthcare team. Acting as a source of clinical and theoretical knowledge for junior staff as well as patients and their significant others, providing support and clinical advice. Perform clinical procedures appropriate to the history and physical examination of the patient, including further invasive testing and treatments requiring highly developed skills and precision, Integrate both pharmacological and non-pharmacological treatment in patient care/management plans. Works in partnership with the existing senior medical, nursing and AHP teams in driving departmental development and change. Recognise when there is a need for escalation of treatment for medical attention and ensure the appropriate health care profession is called to facilitate optimal patient care. Promote a professional and happy environment conducive to high patient and staff morale, achieved by leadership, personal example and direct involvement in the activities of the clinical areas and the service
