Summary

Key Responsibilities Assess, plan, implement, and evaluate individualised nursing care for patients with acute stroke and related neurological conditions. Perform and document neurological observations and stroke assessments, including use of recognised stroke assessment tools and escalation of clinical concerns. Monitor patients receiving specialist stroke interventions and identify signs of deterioration. Administer medications safely and effectively Support patients with activities of daily living, mobility, nutrition, hydration, and communication needs. Collaborate closely with the multidisciplinary team, including physicians, physiotherapists, occupational therapists, speech and language therapists, dietitians, and specialist stroke nurses. Develop, implement, and review patient-centred care plans.Educate patients and families about stroke prevention, rehabilitation, medication management, and lifestyle modifications. Participate actively in discharge planning and ensure smooth transition of care to community services. Maintain accurate, timely, and comprehensive clinical documentation.Supervise and support healthcare assistants, student nurses, and newly qualified staff. Contribute to clinical governance, quality improvement initiatives, audits, and service development. Adhere to infection prevention, safeguarding, risk management, and health and safety policies.
