Summary

Complete medication queries, medicines reconciliation, and repeat prescription processes. Undertake CQC safety searches and coordinate medication recalls as required. Liaise with patients regarding medication queries, usage, side effects, and adherence to prescribed treatments. Support the safe and cost-effective use of medicines in line with current formularies, national guidance, and local policies. Monitor prescribing patterns and support audits relating to medicines optimisation, patient safety, and quality improvement. Work collaboratively with GPs, Clinical Pharmacists, Practice Nurses, and other members of the multidisciplinary team to improve prescribing efficiency and patient outcomes. Support the management and optimisation of repeat prescription systems and processes. Educate and empower patients on self-care, medication use, and treatment adherence. Lead and manage designated areas of the Medicines Optimisation Framework to support PCN and practice objectives. Maintain accurate, timely, and confidential patient records in accordance with professional and organisational standards. Contribute to medicines management projects and initiatives across South Leicestershire Medical Group and Billesdon Surgery.
