Summary

Join the Community Recovery Service and play a vital role in helping adults achieve their rehabilitation goals and regain their independence. Working under the guidance of Occupational Therapists and Physiotherapists, you will deliver person-centred rehabilitation interventions to patients in their homes. As part of a supportive multidisciplinary team, you will contribute to the assessment, treatment and ongoing recovery of individuals with a range of rehabilitation needs.

You will gain experience across a broad range of clinical activities while developing your skills through structured training, supervision and multidisciplinary working.

·      Support the assessment, treatment and rehabilitation of adults receiving care from the Community Recovery Service.

·      Deliver agreed rehabilitation programmes independently within your level of competence, escalating concerns and changes in patient condition appropriately.

·      Assist patients to maximise independence and achieve personalised rehabilitation goals.

·      Assess, fit and demonstrate rehabilitation equipment, ensuring safe and appropriate use.

·      Undertake risk assessments within patients’ homes and during treatment interventions.

·      Participate in home visits and support safe moving and handling practices.

·      Communicate effectively with patients, including those with sensory, cognitive or communication difficulties.

·      Maintain accurate clinical records and documentation using SystmOne.

·      Contribute to multidisciplinary team meetings, clinical governance activities and service objectives.

·      Support a safe, clean and organised clinical environment and participate in service rotas, including weekends and bank holidays as required.
