Summary

Are you an experienced communications and engagement professional committed to patient voice and community empowerment?

Bexley Care Plus, the newly created Integrator in Bexley, is offering a great opportunity to work across organisational boundaries – including the NHS, local government and voluntary sector – to support its aims of understanding and meeting local needs through person-centred, equitable and proactive care at a neighbourhood level.

As our Communications and Engagement Manager, you will develop a comprehensive Integrated Neighbourhood Teams (INT) vision, approach and delivery model; provide education and digital tools to help patients manage long-term conditions; co-design resources with patients and carers; and embed social prescribing to connect people to wellbeing activities.

While you will be part of the London Borough of Bexley’s Communications & Engagement Team - a friendly and professional team with a broad range of skills and experience – your work will take you across the partnership.

Bexley Care Plus (Integrator) brings to together the London Borough of Bexley, Oxleas NHS Foundation Trust, the local GP Federation and the 4 Primary Care Networks. Oxleas NHS Foundation Trust is the ‘host’ NHS organisation.

This is a fixed-term post for 12 months.
