Summary

Supporting individual to access social, leisure, and community activities.

•Encouraging independence and self-confidence.

•Assisting individual to engage with her hobbies and interests, including creative activities, baking, and gaming-related events or groups where appropriate.

•Providing companionship and emotional support in an age-appropriate and respectful manner.

•Understanding and accommodating individuals sensory, communication, and mobility needs.

•Supporting safe participation in activities and outings.

•Helping individual develop life skills and confidence in the community.

Roles and Responsibilities

•Support individual to access community, social, and leisure activities.

•Encourage and promote independence, confidence, and decision-making.

•Build a positive, trusting, and respectful working relationship with individual.

•Accompany individual to activities, events, appointments, and outings as required.

•Support individual to pursue her interests, including writing, arts and crafts, baking, gaming, and other creative activities.

•Assist with planning activities and developing new hobbies and experiences.

•Provide practical support with mobility when required, including assisting with wheelchair-related needs during outings.

•Be aware of individuals health conditions and respond appropriately to any health-related concerns in line with agreed guidance.

•Support individual in managing sensory, social, and communication challenges in different environments.

•Encourage the development of life skills, including community access, money handling, travel confidence, and organisation where appropriate.

•Promote individuals safety, wellbeing, dignity, and inclusion at all times.

•Maintain appropriate communication with her family regarding activities, concerns, and achievements.

•Adapt support according to individuals fluctuating energy levels, pain, fatigue, and health needs.

We require a driver. Individual needs access to the hospital at all times and cannot tolerate public transport.

About Me

Individual is a bright, creative, and determined young person with a great imagination and a range of interests. She is autistic (PDA profile) and has ADHD, alongside several physical health conditions including Hypermobile Ehlers-Danlos Syndrome (hEDS), supraventricular tachycardia (SVT), asthma, dyspraxia, and tics. Individual is an ambulatory wheelchair user and may use her wheelchair to help manage pain, fatigue, and mobility challenges.

Individual enjoys writing stories, arts and crafts, baking, online gaming, and playing Paralives. She loves expressing her creativity and would benefit from support to access activities in the community, explore her interests, and continue developing her independence.

She values people who are patient, understanding, have a good sense of humour, and who can support her without taking away her independence.

She is sarcastic, hilarious, fun to be around, thoughtful and kind. We are looking for someone who can build a positive and trusting relationship with individual, support her to participate in activities she enjoys, and encourage her confidence while respecting her individuality and needs.
