Are you looking for a varied and rewarding role working with people, supporting them to retain or regain independence, where you can make a difference in people’s lives every day?
If so, we are looking to recruit 2 X Wellbeing Worker’s to join our Living Well Support Service team Falls Prevention Winter Project, working across the Central Lancashire area.
The Living Well Support Service supports people to regain and maintain independence, helping to prevent hospital admissions and reduce the need for residential care. In the last year, the service supported over 9,000 people across Lancashire.
This winter, we are expanding the service with targeted support for older people at risk of falls, loss of independence, and avoidable hospital admissions, helping them stay safe and well at home during the winter months. The Falls Prevention Winter Project aims to keep older people out of hospital and reduce the frequent use of urgent and emergency care (UEC) services throughout winter, when vulnerable adults are at increased risk, by providing targeted interventions and support.

About the role:
As a Wellbeing Worker you will:
Support older people who may be at increased risk of falls, loss of independence, or avoidable hospital admission.
*You will undertake person-centred holistic assessmentsto identify risks, unmet needs and develop personalised plans to improve safety, wellbeing and independence.
*Provide continued support for a designated period, to that build upon strengths and achieve personal goals.
*You will use a solution-focused approach to ensure that people can access the help that they need when they need it, either from Age UK Lancashire or from other community-based support.

Please note there is no personal care within this role.
This role is based at an AUKL Preston Base (PR2 area), you will make community visits across the Preston area, working in the community, hospitals and client’s homes. You will be required to attend meetings and spend time at our offices working with Age UK Lancashire colleagues.
