As a
Complex Care Specialist Support Worker, you'll be part of a highly skilled team providing urgent support wherever it's needed across England and Wales. Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance.
This is a role where your experience, compassion and confidence genuinely change lives.
What we can offer you:
Up to £29,000 per year, depending on experience
£3,000 annual car allowance
Paid mileage for business travel
Accommodation provided when working away from home

More about the role:
You'll support the set-up, stabilisation and delivery of complex care packages, including:
Supporting safe and timely hospital discharges
Delivering high-quality, person-centred care
Supporting clients with daily living activities and promoting independence
Building routines that improve confidence and wellbeing
Providing reassurance and continuity for clients and their families
Mentoring and supporting new Support Workers
Working alongside Care Managers and Clinical Professionals
Ensuring medication, equipment and care supplies are in place
Identifying concerns and escalating appropriately
Travel is an essential part of this role:
This is a regional role so you will need to be happy and able to travel across your designated region
This may include:
Travelling to assignments across the country
Working on both live-in and live-out care packages
Staying away from home when required
To support you, we'll provide:
Paid business mileage
Accommodation for assignments outside reasonable travelling distance
Accommodation for live-out packages where overnight stays are required

What we are looking for:
Essential:
Full UK Driving Licence
Experience supporting people with complex care needs
Confidence working independently and adapting to new environments
Excellent communication and organisational skills
A calm, compassionate and professional approach
Flexibility to travel within your regionally
Desirable:
Hospital discharge experience
Level 2 Health & Social Care qualification (or above)
