Summary
The Practice Nurse will be responsible for delivering high-quality, patient-centred nursing care to the practice population, working both independently and collaboratively within the multidisciplinary team. The post holder will provide a range of clinical services, health promotion activities and chronic disease management in accordance with professional standards, practice policies and current evidence-based guidelines. Key Responsibilities Undertake nursing assessments and provide appropriate care, treatment and management for patients in line with their individual needs and own scope of practice. Deliver a range of practice nursing services, including chronic disease management, cervical screening, immunisations and vaccinations, wound care, ear irrigation, health promotion and lifestyle advice. Work collaboratively with GPs, Advanced Care Practitioners and other members of the multidisciplinary team to deliver safe, effective and compassionate patient care. Support the identification of practice population health needs and contribute to the development and delivery of services to meet those needs. Demonstrate and apply knowledge of the Quality and Outcomes Framework (QOF) and support the achievement of practice targets. Provide education, support and self-management advice to patients living with long-term conditions. Offer specialist advice and care to patients with respiratory conditions such as COPD and Asthma and where appropriately qualified, Diabetes. Undertake clinical procedures and investigations as required, within competence and training. Maintain accurate, contemporaneous clinical records in accordance with professional, legal and organisational requirements. Participate in clinical audits, quality improvement initiatives and service development activities. Engage in continuing professional development and undertake training required to support role progression and service needs. Practice at all times in accordance with the NMC Code, maintaining professional standards, accountability and patient confidentiality.
