Summary

Carry out comprehensive Care Act assessments to identify adults’ care and support needs.

Develop, implement and review personalised care and support plans in partnership with individuals, families and carers.

Complete regular reviews of care packages to ensure support remains appropriate and outcomes-focused.

Assess and manage safeguarding concerns, taking appropriate action to protect adults at risk of abuse or neglect.

Work with older people and young adults across a range of complex social care needs.

Support individuals with physical health conditions, acquired brain injuries, mental health needs, learning disabilities and autism.

Apply the principles of the Mental Capacity Act 2005 when assessing capacity and making best-interest decisions.

Undertake Community Deprivation of Liberty Safeguards (DoLS) work and ensure legal requirements are followed.

Support Court of Protection matters and provide appropriate professional assessments and reports where required.

Work effectively within multi-disciplinary teams alongside health professionals and other partner agencies.

Assess and arrange appropriate care and support services, including consideration of personal budgets.

Prepare and submit budget requests and ensure care packages provide appropriate and cost-effective support.

Maintain accurate, timely and confidential case records, assessments, reviews and professional reports.

Manage an allocated caseload effectively, prioritising urgent and complex cases while working within relevant legislation, policies and professional standards.
