Summary
This job supports older adult patients living in the community who need support with daily living after they are discharged from hospital. This supports people with long‑term and short-term health conditions or those recovering from illness by delivering person‑centered care in their home.
Current requirements for Carer's: Living Care, respite care, and Home Care.
Location: Leeds, Shipley, Gosforth, Castleford, Wakefield, Batley, Bradford, and Huddersfield.
The role focuses on promoting independence, dignity, and quality of life, while ensuring safety and wellbeing. The carer will provide long-term and short-term person-centred support to individuals living at home and who have recently been discharged from hospital or are looking for daily or ongoing care living support. The role also focuses on rehabilitation, stabilising daily routines, preventing readmission, promoting independence, and ensuring the individual can safely manage at home while recovering from illness, surgery, or a medical event.
