Summary

Provide high quality nursing care for children and young people in the London borough of Newham, which is planned and delivered, reviewed and evaluated within the clients home, wound dressing clinic or other appropriate setting within the community using family centered, evidence based practice. Assess and order equipment and supplies for the child or young person to enable them to manage their condition safely in the community setting, including regular reassessments dependent on the changing needs of the child/young person. Provide initial triage and assessment of new referrals to the service, identifying where urgent response is required and allocating to staff, based on staff logistics and skill mix. Initiate and manage safe, effective discharge from hospital to home carrying out risk assessments of the receiving community setting. Enable and promote independence by teaching, supporting and encouraging carers and children, including those children with complex health needs, to participate in meeting their own care needs where possible in community settings. Initiate and manage appropriate changes in care in negotiation with children, families and professionals following review of planned care at either case review, morning handover or joint visits. Provide emotional support to clients and families who may be facing difficult decisions, bad news or bereavement. Communicate with families for whom English is not their first language and/or children with communication difficulties/ special needs. This involves working in collaboration with health advocates (interpreters), or using highly developed verbal/ non-verbal communication skills
