Summary

The Senior Practice Governance Facilitator support the delivery of patient safety, practice governance and quality improvement priorities across Adult Community Services. Working closely with the Head of Nursing and in collaboration with the Divisional Director, Clinical Director, Heads of Service, Professional Leads and operational teams, the post holder will support services to embed learning, improve practice and provide assurance on the quality and safety of care. The role will coordinate learning from incidents, complaints, audits, quality assurance visits and Patient Safety Incident Response Framework (PSIRF) learning responses, ensuring actions are implemented and improvements embedded into everyday practice. The post holder will monitor governance and quality improvement action plans, analyse quality and safety data, identify trends and risks, and support services to deliver continuous improvement initiatives. Key responsibilities include supporting divisional governance processes, facilitating clinical audits, preparing reports and assurance updates, providing specialist advice on governance and quality improvement matters, and supporting teams to respond to regulatory and quality assurance requirements. The post holder will also contribute to staff training, promote the sharing of learning and best practice, and support the involvement of service users and carers in service improvement activities. Car driver essential as a majority of the posts are community based, the post holder will need a full valid driving licence and access to a car to use regularly for business purposes including business insurance (unless you have a disability as defined by the Equality Act 2010).
