Summary
We have an exciting opportunity for a Bengali/Sylheti speaker to work as a dementia care advisor/navigator within Tower Hamlets post dementia diagnostics service. Consisting of a band 6 post diagnostic Mental Health Nurse and 2 other dementia care advisors/navigators.
The aim of the service is to provide post diagnostic information and support to enable people with dementia and their families prepare for what to expect as the illness progresses and get the support they need.
The team sits within the broader Integrated Care Mental Health team where patients can be escalated too if they require more intensive support.
Work with individuals and their families, to provide information and signposting and onward referrals to support the person to live as best they can with the illness. This may include referrals to the carers centre, Adult Social Care, telecare, among others. We work closely with the Alzheimer Society who run social inclusion groups in the borough. Working with the patient and their carers on a personalised care plan and reviewing before discharging back to the GP.
To facilitate Bengali Cognitive Stimulation Group.
To offer and facilitate GRACE interventions.
Work with the wider MDT.
Attend Dementia Support Service team meetings.
Keep up to date notes on Rio.
Ensure supervision, appraisal and Stat Mandatory training are up to date.
You will be required to visit people in their own homes in Tower Hamlets.
