Summary

Provide practical and emotional support to children, young people, and families to promote their wellbeing, independence, and positive outcomes.

Work closely with families to identify their needs, develop support plans, and help them access appropriate services and community resources.

Build positive, trusting relationships with children and families while maintaining professional boundaries at all times.

Support parents and carers in developing parenting skills, improving family relationships, and creating a safe and nurturing home environment.

Carry out home visits and community-based support sessions, ensuring all interactions are recorded accurately and confidentially.

Monitor the progress of children and families, reporting any safeguarding concerns or changes in circumstances to the appropriate professionals.

Work collaboratively with social workers, schools, health professionals, and other partner agencies to deliver coordinated support.

Maintain accurate records, case notes, and reports in line with organisational policies, procedures, and data protection requirements.

Promote equality, diversity, and inclusion by treating all children, families, and colleagues with dignity, respect, and fairness.

Follow all safeguarding, health and safety, and organisational policies, ensuring the welfare and safety of children and vulnerable individuals remains the highest priority.
