Summary

To deliver a high standard of patient care as a healthcare assistant in the community and report any significant outcomes or changes in conditions to senior team members. To provide support within the team and work as a team player. Responding effectively to patient needs and ensuring patient choice and ease of access to services. To work within a competency framework appropriate to the individuals scope of practice. To undertake clinical procedures, as delegated by senior staff, such as wound care, pressure ulcer care and management, equipment ordering, leg ulcer care, catheter changes and on-going care. To advise carers and/or patients how to manage care in between visits and promote health education on an individual basis as directed by senior staff.
