Summary
To be responsible for the operational management and coordination of the Community Nursing teams attached to a number of GP practices across a locality/neighbourhood, ensuring effective and efficient delivery of a high quality timely service. To assist the Operational Manager in maintaining operational, clinical, professional, organisational and national objectives. The post holder has the autonomy to act independently and is responsible for leadership, maintaining and improving all aspects of clinical governance, evidence based practice and quality assurance. To undertake highly complex assessments. Duties and Responsibilities: Clinical: To perform comprehensive assessment of highly complex patient nursing needs. Plan, assess, implement and evaluate care delivery according to changing health needs using prescribing skills To ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care To actively contribute to multidisciplinary meeting as appropriate To establish and maintain effective communication with individual groups and communities in complex and potentially challenging situations To work collaboratively with the Primary Care teams other professional and agencies to ensure patients needs are met To involve patients and carers in the planning and delivery of care To competently perform a range of clinical skills related to the post To maintain accurate records in accordance with NMC standards To identify skills and knowledge to address self and individuals training needs by undertaking PDPs under the KSF framework To participate in the health needs assessment of practice populations to identify areas of unmet needs To demonstrate the ability to work without direct supervision and to solve problems Provide advice, guidance and support to District Nursing staff on all clinical and professional issues, e.g. vulnerable adults, legal and ethical issues in nursing. Act as an agent for highlighting and disseminating best practice in nursing. Supervise and monitor professional standards as laid down by the Nursing & Midwifery Council (NMC). Ensure record keeping standards are met in accordance with the NMC Guidelines for Records and Record Keeping. Managerial/Leadership: To manage time to ensure that 80% of time is spent on managerial duties and 20% clinical. To be responsible for the equitable distribution of workloads across teams Manage the District Nursing Services in a locality through the development and supervision of staff in order to maintain an efficient service and ensure District Nursing teams and caseloads are managed effectively. Lead, implement and monitor, within the locality, developments in the District Nursing service that are in line with local and national directives. Lead and chair working groups specific to the District Nursing service. To lead, within the locality, in multi-professional negotiations/presentations involving the development of the service and to support an integrated, seamless service across the acute and primary care trusts, with Matrons and Social Services. Take designated lead responsibility for all of the following at locality level; education, audit, Out of Hours, treatment room services, phlebotomy services and Clinical Risk Management. Develop partnership approaches with other agencies/services to inform and enhance service delivery. Represent the service at multi-agency and multi-disciplinary forums and facilitate/chair as appropriate. To have responsibility for managing projects within the District Nursing Service. Monitor service delivery and highlight resources required to respond to the changing needs of the service. Ensure that appropriate information is available and evidenced to the Operational Manager to inform the strategic development plan. To provide advice and support to the Operational Manager to inform strategic projects, future service provision and business planning for District Nursing Service, i.e. provide business plans for extension of new and existing services. Manage all personnel issues including complex personnel issues within own area of responsibility, i.e. capability, conduct, behaviour, fraud, managing attendance. Manage flexible working and innovation appropriate to practice. Lead the recruitment process, interview and select staff and chair interview panels. Actively participate in retention and professional development of staff. Manage staff development and identify training needs through Personal Development Plans, linked to KSF. Through the appraisal process and other systems monitor the performance of staff. Utilise information to devise and monitor action plans to inform the training agenda through the Personal Development monitoring process. Ensure induction, preceptorship and mentorship programmes are implemented within area of responsibility. Including development of competency packs. Develop and implement Standard Operational Procedures (SOP). Implement Trust Policies, Procedures and ensure staff adherence through education and audit. To ensure compliance of self and others with Health and Safety Policies and provide a level of knowledge, training and support which contributes to providing a safe working environment and meets the requirements of the Health & Safety at Work Act, i.e. essential steps auditing, PDR, manadatory training. To manage prompt recording and reporting of accidents/incidents. Investigate accidents, formal and informal complaints and untoward incidents in accordance with Trust policy and produce, implement and monitor action plans, as appropriate. Reporting to the CCG panel as required. Undertake investigation on pro-active management of individual and team performance issues within the locality, including grievance and harassment, poor performance, conduct and ill-health; seeking HR and line management support as necessary. Ensure staff compliance with recording and monitor activity patterns in order to inform decision making re service delivery, allocation of resources, skill-mix, etc. Ensure that all communication systems are effective. To take the lead role in maintaining and improving all aspects of clinical governance, evidence based practice and quality assurance. Identify and implement change and use management and leadership skills to initiate improvements in practice. To ensure timely accurate information is provided on annual leave, expenses and study leave. Responsible for work force planning ensuring safe staff levels within locality. Authorised signatory for mileage, annual leave and study leave, ensuring staffing levels are safe. Manage and maintain ESR data base including all staff activities. To monitor and report to Operational Manager any general HR issues discussing and taking action as appropriate To actively manage sickness and absence of staff accessing support from the Operational Manager as applicable. To ensure effective use of physical and financial resources including the ordering of supplies and equipment To ensure that weekend working is coordinated across the individual division To ensure as far as possible that the working environment is safe and staff adhere to loan worker policy To provide leadership and support by promoting professional development and encouraging clinical supervision To contribute to the development of the district nursing service To oversee the collection of all monitoring data and contribute to performance management Deputise, in the absence of the Operational Manager, in operational and professional issues within District Nursing Service. Identify and implement change and use management and leadership skills to initiate improvements in practice. Monitor locality budget and ensure resources are utilised effectively. Manage and encourage audit activity, ensuring development of action plans where required and re-audit. Triangulation audits. Revalidation confirmer status for all registered staff within locality.
