Summary

Staffordshire and Stoke-on-Trent Integrated Care Board. The Patient Flow Coordinator will play a pivotal role in supporting the efficient operation of inpatient services by facilitating effective discharge planning, patient progression and timely transfers of care. Working closely with multidisciplinary teams, operational managers, community services and external partners, the post holder will support the delivery of safe, person-centred and outcome-focused care while ensuring patients experience timely discharge from hospital when clinically appropriate. The post holder will be responsible for coordinating processes that support patient flow across inpatient services, with a particular focus on reducing unnecessary length of stay, preventing delayed discharges and ensuring compliance with Trust policies, national guidance and best practice principles. Patient Flow and Discharge Coordination *Coordinate board rounds, MDT meetings and patient flow discussions, ensuring discharge plans, expected discharge dates and actions are progressed and escalated as required. Work with ward teams and partner organisations to overcome discharge barriers, support D2A pathways, reduce delays and improve patient flow and patient experience. MPFT Choice Policy Support the MPFT Choice Policy, ensuring patients, relatives and carers receive information to make informed discharge decisions. Monitor compliance, support actions to address delays, and ensure patient choice discussions and outcomes are accurately recorded. Escalation and Operational Support Monitor patient progression and discharge plans, escalating delays and supporting timely, safe discharge. Contribute to patient flow and capacity meetings, working with teams and partners to resolve discharge barriers and support operational decisions. Winter Pressures and Capacity Management Support winter pressures and discharge initiatives by monitoring capacity, patient flow and discharge performance. Work with partners to coordinate discharges, escalate risks and support surge plans to maintain patient flow. . Data, Reporting and Governance Maintain patient flow records and discharge data, producing reports to support performance monitoring and decision-making. Support audits and service improvements, ensuring compliance with Trust policies, Information Governance and GDPR. Communication and Working Relationships Build effective relationships with teams, patients, carers and partners to support communication and discharge planning. Promote a collaborative, patient-centred approach to patient flow, reflecting Trust values in all activities. Role Summary This role is integral to supporting the Trust's objectives to improve patient flow, reduce unnecessary length of stay and ensure timely, safe and effective discharge from inpatient services. The post holder will have a key role in coordinating discharge planning activity, supporting LOS reviews, facilitating board rounds, embedding the MPFT Choice Policy and contributing to winter resilience and escalation processes. Through close partnership working with clinical teams and system partners, the post holder will help ensure patients receive the right care, in the right place, at the right time.
