Summary

Job Purpose The Clinical Transformation Lead is an experienced General Practitioner who will provide dedicated clinical leadership for the transformation and redesign of services within the practice. This role is a fixed-term post established to deliver a programme of clinical transformation, service improvement, workforce redesign, digital innovation, and quality improvement. The postholder will be relieved of all routine patient-facing clinical duties for the duration of the appointment to enable full focus on the transformation programme and delivery of strategic objectives. Working closely with the Practice Leadership Team, Transformation Lead, PCN, Integrated Care Board (ICB), and wider system partners, the postholder will ensure that all transformation activity is clinically led, evidence-based, safe, and aligned with national and local priorities. The Clinical Transformation Lead will champion modern models of care, support workforce optimisation, improve patient access and experience, strengthen governance arrangements, and embed sustainable changes that improve quality, productivity, and outcomes. Key Responsibilities 1. Clinical Leadership for Transformation Lead the clinical aspects of the practice transformation programme. Provide strategic clinical oversight and assurance for all service redesign initiatives. Ensure transformation plans are clinically safe, evidence-based, and aligned with best practice. Act as the senior clinical advisor to transformation and service improvement projects. Support the development of innovative care models that improve access, quality, and efficiency. Represent the practice in clinical discussions with system partners and external stakeholders. 2. Service Redesign and Improvement Lead the clinical redesign of patient pathways and models of care. Review existing services and identify opportunities to improve quality, productivity, and patient outcomes. Support implementation of multidisciplinary and neighbourhood-based working models. Ensure transformation programmes deliver measurable improvements in access, experience, and clinical effectiveness. Promote population health management approaches within service redesign activity. 3. Workforce Transformation and Clinical Skill Mix Support clinical workforce redesign in partnership with the Transformation Lead and practice leadership team. Support the development of effective multidisciplinary teams, ensuring clinicians work at the top of their licence. Promote appropriate utilisation of Advanced Nurse Practitioners, Clinical Pharmacists, Paramedics, Physician Associates (where appropriate), and ARRS staff. Identify opportunities to reduce GP workload through pathway optimisation and role redesign. Support succession planning and development of future clinical leaders. 4. Patient Access and Experience Provide clinical leadership for improvements in patient access. Review and redesign clinical triage, same-day access, continuity of care, and appointment systems. Ensure access models remain clinically safe, equitable, and responsive to patient need. Use patient feedback, complaints, incident reviews, and outcome data to inform service improvement. Support implementation of innovative access solutions, including digital-first approaches. 5. Clinical Quality, Governance and Risk Management Strengthen clinical governance frameworks across the practice. Ensure transformation programmes comply with CQC requirements, NHS contractual standards, and professional guidance. Lead clinical risk assessments associated with service change. Oversee clinical audits, quality improvement programmes, and learning from significant events. Promote a culture of continuous improvement, patient safety, and shared learning. 6. Financial Sustainability and Service Development Provide clinical leadership to support financial recovery and sustainability initiatives. Contribute to opportunities for income maximisation through optimisation of QOF, LES, and other commissioned services. Assess the clinical impact of proposed efficiency initiatives and service developments. Support business case development for new and redesigned services. Ensure value for money while maintaining quality and safety standards. 7. Digital Transformation and Innovation Act as the clinical lead for digital transformation projects. Support implementation of digital tools, online consultation platforms, artificial intelligence applications, and workflow automation. Ensure technology adoption is clinically safe and delivers tangible benefits for patients and staff. Promote digital innovation that improves efficiency, productivity, and patient experience. Support staff engagement and confidence in the use of new technologies. 8. Stakeholder Engagement and System Leadership Develop effective relationships with practice staff, patients, the Patient Participation Group, PCN, ICB, community providers, and secondary care partners. Provide clinical leadership within neighbourhood and place-based transformation programmes. Promote collaboration across organisational boundaries to improve integrated care pathways. Represent the practice at relevant clinical, operational, and transformation forums. Influence system-wide developments impacting primary care. 9. Change Management and Staff Engagement Act as a visible clinical leader throughout the transformation programme. Support staff through organisational change and service redesign. Promote engagement, communication, and collaboration across all teams. Help develop a positive culture focused on continuous improvement and innovation. Support clinical leadership development within the wider workforce.
