The Role

The practice is seeking an enthusiastic and motivated Practice Nurse to deliver a high standard of nursing care across a varied patient population. Working alongside experienced GPs, Advanced Nurse Practitioners, Healthcare Assistants, Clinical Pharmacists and the wider multidisciplinary team, the successful candidate will play a key role in delivering both routine and chronic disease management services.

The post holder will provide evidence-based nursing care, promote health and wellbeing, and contribute to the continued delivery of safe, effective and compassionate primary care services.

This role would suit an experienced Practice Nurse looking to join a progressive practice where professional development is encouraged and clinicians are given the time and support to provide outstanding patient care.

Key Responsibilities

    • Deliver high-quality nursing care to patients within the practice.

    • Undertake chronic disease management clinics, including Diabetes, COPD, Asthma and Hypertension.

    • Perform cervical cytology (smears).

    • Administer childhood, adult and travel immunisations and vaccinations.

    • Carry out wound care, dressings and suture removal.

    • Conduct NHS Health Checks.

    • Perform ECGs, blood pressure monitoring and spirometry where appropriate.

    • Undertake venepuncture and other routine clinical procedures.

    • Deliver health promotion and lifestyle advice.

    • Support patients with long-term condition management and preventative healthcare.

    • Maintain accurate and contemporaneous clinical records.

    • Participate in infection prevention and control activities.

    • Contribute to clinical audits, quality improvement initiatives and practice development.

    • Work collaboratively with the wider multidisciplinary team to ensure the highest standards of patient care.

    Candidate Requirements

    The successful candidate will possess:

      • Registered Nurse with a valid NMC registration.

      • Previous experience working as a Practice Nurse within primary care.

      • Experience managing long-term conditions such as Diabetes, COPD and Asthma.

      • Competency in cervical cytology.

      • Experience delivering routine immunisations and vaccinations.

      • Excellent communication and interpersonal skills.

      • A compassionate, patient-centred approach to care.

      • Ability to work both independently and as part of a multidisciplinary team.

      • Strong organisational and time management skills.

      • Experience using EMIS Web or SystmOne would be advantageous.
