Summary
Help People Make a Safe Transition from Hospital to Home
Disability Can Do are seeking a compassionate and motivated Hospital-to-Home Community Connector to deliver an exciting new pilot project supporting disabled adults and older people across Caerphilly County Borough.
This role will provide short-term practical support to people returning home from hospital who need a little extra help to regain confidence, remain independent, and avoid unnecessary crisis or readmission.
Working directly with individuals in their homes and communities, you will provide practical assistance, emotional reassurance, and connections to ongoing support services.
You will also contribute to and support the development of local partnerships and referral pathways to ensure the pilot is well integrated within health, social care and community services and contributes to the longer-term sustainability of the service.
What You'll Do
- Visit people recently discharged from hospital.
- Help identify immediate practical needs.
- Support with shopping, prescription collection and other everyday tasks.
- Signpost and refer individuals to appropriate services.
- Work closely with local health, social care and voluntary sector partners.
- Monitor outcomes and help demonstrate the impact of the pilot.
