Summary

Job Purpose Summary To provide a dietetic service to adults as part of the Obesity Pathway Innovation Programme across Lincolnshire, working within a community-based weight management service. To support colleagues in the provision of a dietetic service in clinics in the community, using evidence-based practice and conforming to best practice guidelines, including NICE NG246 (Overweight and obesity management) and NICE TA1026 (Tirzepatide). To play an active role in shared, MDT-based decision-making regarding weight management medication (including tirzepatide and other GLP-1/GIP-based therapies), providing dietetic input to initiation, monitoring and review decisions. To co-design the OPIP curriculum and engage a target patient cohort in co-production, particularly during the premobilisation phase of the programme. To liaise with clinical and non-clinical professionals, patients, and family/carers regarding nutrition. To provide support and professional supervision of health coaches, and to contribute to the leadership of dietetic service development within OPIP (see Section 3). 2. Responsibilities / AccountabilitiesOperational Delivery To organise a daily clinical caseload and base practice on clinical priorities/referral criteria, standards and protocols. To be responsible for designated community caseloads. To be responsible for designated outpatient clinics/group talks for patients who have complex medical conditions. To provide leave cover when other members of the dietetic team are absent, according to clinical competencies and on an agreed cover rota. To be a core member of the MDT, contributing dietetic assessment and input to shared decisions on treatment Patient / Customer Care To assimilate and analyse complex patient information from a variety of sources, including health records including a patient's diagnosis and the results of any investigations. To undertake comprehensive nutritional assessments of patients and make clinical judgements on a range of complex dietetic problems requiring investigation, analysis and assessment. To formulate individualised management and treatment plans autonomously, using clinical reasoning and a wide range of treatment skills and options. To be responsible for the formulation of dietetic advice to ensure a high standard of clinical care. To utilise developed communication and teaching skills in the adjustment of care plans, negotiating with the patient/client/carer using motivational communication skills to maximise patient engagement and compliance. To acknowledge possible communication barriers and tailor dietetic advice and supporting information to the patient/client's needs. To contribute dietetic and nutritional expertise to MDT/shared decision-making on the prescription, initiation and ongoing review of weight management medication, in line with ICB guidance, NICE NG246 and NICE TA1026. To monitor patients' nutritional adequacy during pharmacotherapy-assisted weight loss in line with NICE guidance on prescribing, reviewing and stopping weight management medication. To monitor patient progress using a variety of sources including nutritional assessment, biochemistry, anthropometry and food record charts, and use these to review and alter treatment as appropriate. To assess drug treatment plans and amend dietary intervention as appropriate. Service Development and Improvement To lead, with colleagues, the co-design of the OPIP curriculum and to engage a target cohort of patients in structured co-production activity, particularly during the premobilisation phase. Along with other dietitians, to evaluate and update nutritional resources and patient literature. To contribute data and evaluation input to support OPIP's programme monitoring and reporting obligations (OPIP is Innovate UK-funded and subject to evaluation requirements). To undertake professional and educational activities to develop knowledge and maintain professional/clinical standards, including external courses, research, professional group membership and clinical update sessions. To take part in the annual appraisal process and develop a personal development plan. To attend clinical, service and statutory meetings as appropriate, including clinical governance meetings. To participate in the Nutrition and Dietetic Service arrangements for practice supervision. To critically appraise and review current literature on relevant topics. To consider clinical and non-clinical risk management as part of day-to-day work and complete incident report forms as appropriate. To participate in audit to ensure best practice, organisational policy and key performance indicators are adhered to. To be legally responsible and accountable for all aspects of professional practice, adhering to HCPC guidelines. To comply with organisational and departmental policies and procedures. To promote the health of patients, carers and members of the public. To network with dietitians across the organisation-wide Nutrition and Dietetic Service. To assist in the induction of new staff members. Information Management To maintain accurate, comprehensive and up-to-date documentation in line with national and local record-keeping standards, and to communicate assessment and treatment results to appropriate disciplines via letters and reports. To collect accurate statistical information to assist in the monitoring and development of the Nutrition and Dietetic Service, and to meet the requirements of the organisation, commissioners, and OPIP's Innovate UK funding evaluation. To be competent in the use of information technology, including nutritional software, to assist with nutritional assessments. Finance and Resource Management To maintain stocks of dietary education materials, resources and dietary literature as appropriate. To be responsible for the safe transport and use of IT equipment and anthropometric measuring devices. To participate in the responsible use of nutritional support products in all settings, adhering to ICB guidance. 3. Additional Responsibilities The following apply where the post is appointed at the higher end of the salary range, reflecting greater seniority, experience and managerial responsibility: Support and supervision of OPIP health coaches, including supervision, and input to appraisal and development. Leading, rather than solely participating in, OPIP dietetic service development and quality improvement work, and taking day-to-day responsibility for related budget lines (e.g. dietary resources, nutritional support products). Deputising for the OPIP dietetic/clinical lead as required. Leading co-design and co-production activity with the target patient cohort during premobilisation, including planning and facilitating structured patient involvement sessions. Acting as a senior clinical resource within the OPIP MDT for complex shared decision-making on weight management medication. 4. Effort, Physical Skills and Working ConditionsPhysical skills To undertake accurate anthropometric measurements. To possess keyboard skills. Physical effort To work in a variety of settings which require light physical effort. To carry and transport resources and equipment (e.g. service user information, leaflets, IT equipment, weighing scales and other anthropometric measuring devices). To provide a dietetic service to clients in a variety of settings, which requires lone working, frequent driving, and occasional exposure to unpleasant working conditions. Mental effort To have the ability to concentrate for prolonged periods of time, including in environments with competing demands for attention. Emotional effort To provide a dietetic service in situations with frequent exposure to distressing or emotional circumstances. Working conditions To provide a dietetic service across areas that may involve occasional exposure to unpleasant working conditions, and that requires driving between different locations.
